1. Obtain a sample using a curette or other debridement tools. A sample can also be obtained utilizing a sterile cotton-tipped applicator if the tissue is not dry or hardened.
2. Place the sample (tissue or entire applicator tip) into the provided 2 ml plastic container.
3. Peel off the label from the order form and apply it to the sample container (make sure the writing is visible).
Moist Samples are preferred.

1. Drop the prepared sample container into the front pocket (between the black hashed lines) of specimen transport bag.
2. Fold the diagnostic order form in quarters and place it into the back pocket of the specimen transport bag.
3. Next, peel the white strip off of the bag to expose the adhesive backing.
4. Then, close the flap over the mouth, lining the adhesive up with the 2 black, hashed lines to create a continuous seal.
1. To assemble the Category B shipping box, begin to fold the top up.
2. The side with 4 holes is the bottom.
3. Fold small flaps in (2) so they are hovering over the holes.
4. Next, fold the side up and into the box (3) to hold the small flaps in place.
5. Press the sides down slightly (4) so there are 2 fold edges (inside top and outer top) and the notches go into the four holes in the bottom.
1. Now place the prepared specimen bag containing the sample and order form (5) into the box.
2. Next, fold the side into the interior of the box (6).
3. Push the top down (7).
4. Finally, fold the tabs into the box and push the front in (8). Also, please tape the front shut for extra security.
NOTE: More than one specimen transport bag can be placed in one shipping container. However, only one sample and one order form should be placed in a specimen bag.
Now that your box is prepared (and the FedEx label is already on the box) place the box in the FedEx UN3373 Pak and you are ready to ship it.

Alternatively, you can simply drop off the labeled box at any FedEx / Kinkos location.
Please note: Cat B samples may NOT be placed in a FedEx drop box. Only pick up or direct drop off.
